Schedule a 15-minute introductory call

During this  call, our care navigator will learn about you and your priorities and answer your questions to ensure our services align with your goals.

Complete your computer-based assessments

Before your initial visit, you will complete a set of FDA-approved cognitive assessments on your computer or tablet.

Meet with a dementia prevention clinician

You and your Sunday Health clinician will review your medical history, medications, and risk factors, and complete additional cognition and lifestyle assessments.

Receive actionable medical and lifestyle guidance

Your Sunday Health clinician will prepare a report outlining your modifiable risk factors and clear next steps to optimize your cognitive health.

Can I use my HSA or FSA cover the cost of this service?

You may use your Flexible Spending Account (FSA) or Health Savings Account (HSA) to pay

However, all patients are responsible for working with their FSA/HSA administrator to confirm that the service fall into the plan’s allowable expenses (“qualified medical expenses”).
